Depending on the results of the assessment, you can get free help to improve your health.

This includes:

• stopping smoking
• having a healthy weight
• physical activity programmes
• NHS health checks

You can complete our simple request form, below, to ask for an assessment.

Alternatively, your GP or health professional can use the form below to refer you to support services.

We also receive referrals from the Essex County Fire and Rescue Service Safe and Well team.
